Risk assessment is a systematic process of identifying potential hazards, its likelihood and severity, and controlling risks.
The risk assessment may be qualitative or quantitative. In qualitative risk assessment score of risk is calculate using terms such as low, medium or high while in quantitative risk assessment score of risk is calculated using numbers (1, 2, 3, …).
This risk assessment template is quantitative risk assessment form which also include risk assessment matrix. Following information may be included in the risk assessment form.
Area, Activity/Process, Assessment Date, Activity hazards, consequences, Likelihood, Severity, Risk score, Existing controls, Required controls, Actions to be taken, Planned target date, and compliance date.
