An incident report form is used to keep the details of an incident that occurred in the workplace. It also reveals the circumstances that caused the incident. The same will help in preventing future incidents.
The following should be reported in every incident report.
- Type of Incident (Near-miss or accident)
- Location
- Date and Time
- Incident Description
- Name of the person involved
- Name of witnesses (in case of an accident)
- Follow-up Action
